Birth Certificate Requests
Overview
A birth certificate can be obtained in person at the City Clerk's office or by mail. Only the mother and/or father listed on the birth certificate. or the child (child must be 18 years of age or older) may obtain these records.

Legal guardians must present a current certified copy of the court order in order to obtain a child's birth certificate.

Applying In Person

The applicant must bring current official photo identification in the form of a valid U.S. driver license, non-driver ID card (issued by Motor Vehicles Only), or a passport.

Applying By Mail
The applicant must send in the following:
    • A copy of any of the photo IDs listed above
    • Application must be filled out & Signature must be notarized.
    • A self-addressed, stamped envelope
The request may be made using the Birth Certificate Application or on personal stationary.
